Volunteer Application - Affidavit Form
Providing a safe and secure learning environment for students and staff is a district priority. In keeping with this standard, the Board of Education has adopted policies and regulations governing the volunteer process.
What is the process to become a district volunteer? Volunteers must first go through an application process that also includes a background check. To be a volunteer in a District activity (classroom or extra-curricular), a volunteer must sign both sides and complete Parts A and B of the Volunteer Application - Affidavit Form (attached below) and submit a copy of their driver's license for an initial background check. Once this is completed, a volunteer's name is placed on the next available Board of Education agenda for approval. A background check is considered good for a five-year period. If a volunteer's initial background check was in 2010 or 2011, they will need to submit an application for a new background check to update their status.
Once a volunteer has been background checked and approved, they need only to sign and date a yearly Affidavit (sign page one and complete Part B of the form) and submit it to the Board of Education office to remain on the approved list of volunteers for that coming or existing school year. Volunteer status can be activated or reactivated at any time during the school year.
Volunteers can now check their status including their initial background check date and their last submitted affidavit date to maintain their active status. The Approved Volunteer List as well as the Volunteer Application - Affidavit form are attached at the bottom of this page.
