Below please find information regarding a variety of forms available for your convenience and use.  Forms that are available for download at the bottom of this page include the Bobcat Award Nomination, Facility Use Application, Planned Absence Form, Release of Information, Student Acceptable Use Policy, and Volunteer Application - Affidavit. 

Volunteer Application - Affidavit Form

Providing a safe and secure learning environment for students and staff is a district priority.  In keeping with this standard, the Board of Education has adopted policies and regulations governing the volunteer process.

What is the process to become a district volunteer?  Volunteers must first go through an application process that also includes a background check.  To be a volunteer in a District activity (classroom or extra-curricular), a volunteer must sign both sides and complete Parts A and B of the Volunteer Application - Affidavit Form (attached below) and submit a copy of their driver's license for an initial background check.  Once this is completed, a volunteer's name is placed on the next available Board of Education agenda for approval.  A background check is considered good for a five-year period.  If a volunteer's initial background check was in 2010 or 2011, they will need to submit an application for a new background check to update their status.

Once a volunteer has been background checked and approved, they need only to sign and date a yearly Affidavit (sign page one and complete Part B of the form) and submit it to the Board of Education office to remain on the approved list of volunteers for that coming or existing school year.  Volunteer status can be activated or reactivated at any time during the school year. 
 
Volunteers can now check their status including their initial background check date and their last submitted affidavit date to maintain their active status.  The Approved Volunteer List as well as the Volunteer Application - Affidavit form are attached at the bottom of this page.   This listing will be updated and posted on a monthly basis.  If you have questions or need to check a status that may be in progress and does not yet appear on the updated list, then please contact Hayley Head, Executive Assistant to the Superintendent, at hayley.head@ghcsd.org or (614) 485-4015.

The Grandview Heights Schools is synonymous with excellence.  The district's faculty, staff, and community members have earned the district this reputation.  Each employee and countless community members contribute to the overall success of the district each and every day.

To recognize the outstanding contributions of  the many people who make the istrict successful, the Tri-Village Rotary Club has established, and is the sole sponsor of, The Bobcat Award.  This award is presented at a regularly scheduled Board of Education meeting to an individual, or individuals, who have made a noteworthy contribution to the district.  Please download the nomination form to learn more.
